Vertrue Employee Benefits

The following summarizes the range of benefits available to you. These benefits work for you and your family while you work for Vertrue.

In order to be eligible to participate in the insurance plans, you must be a full-time, regular status employee working 35 hours per week or more. For full-time, hourly employees, your coverage will begin on the first day of the month following 90 days of service. For salaried employees, your coverage will begin upon date of hire.

Vertrue pays the majority of the cost of medical and dental insurance for its employees. Your share of the health and dental premiums will be paid with "pre-tax" dollars through convenient, payroll deductions. This provides savings to you by deducting your insurance premiums before payroll taxes are calculated.

MEDICAL INSURANCE

Vertrue offers you and your family a choice of Medical insurance coverage through United Healthcare. You may choose from the Select Plus Plan (Point of Service) or the Preferred Provider Organization (PPO). Both plans offer you the savings of in-network benefits and the freedom and flexibility of out-of-network benefits. Within the benefit is a Prescription Drug Program. This program allows you to choose the most convenient way to fill your pharmaceutical needs.

DENTAL INSURANCE

Our Dental plan allows you and your family to see any dentist you choose and receive benefits equal to a percentage of covered expenses after satisfying an annual deductible. However, by using an in-network provider with United Healthcare, you will see a savings in the negotiated rates for each service you receive. The plan covers preventative care, general and major services, and orthodontia for dependent children under the age of 19.

VISION INSURANCE

Vertrue truly understands the importance of annual eye exams, as exams not only ensure healthy vision, but also can help detect many medical problems early and can affects ones overall health. The Vision program brings access to more than 30,000 private practitioners and optical retailers through the EyeMedNetwork for you and your family. This voluntary benefit is 100% paid by the employee.

LIFE INSURANCE

Vertrue provides regular, full-time employees with a Group Term Life and Accidental Death & Dismemberment insurance equal to one or two times (based on employment level) your annual salary. This company-paid group life insurance is provided through The Hartford. In addition to the company-provided life insurance, we offer two supplemental policies: Supplemental Life Insurance and Voluntary Accidental Death & Dismemberment. Coverage is available for you and your family. Premiums are taken on a bi-weekly basis through payroll deductions.

FLEXIBLE SPENDING ACCOUNTS

All full-time employees are eligible to participate in Flexible Spending Accounts for Medical and/or Dependent Care expenses. Flexible Spending Accounts allow you to pay for non-reimbursed medical and daycare expenses with pre-tax dollars.

DISABILITY INSURANCE

Short and Long-Term Disability insurance is provided to full-time employees at no cost after the eligibility period is satisfied. You may receive up to 60% of your income after a one week waiting period. Based on employment level, you maybe eligible for up to 100% of your income. Short-term disability is paid bi-weekly through payroll deductions and The Hartford administers the long term disability benefit.

401(K) RETIREMENT SAVINGS PLAN

Vertrue offers you a 401(k) savings plan with a company match to assist you in saving for you retirement and attaining your financial goals. All employees, age 18 or older, are eligible to participate on the first of the month following 90 days of employment. You may contribute 1% to15% of your earnings to the Plan on a pre-tax basis. In addition, you have numerous funds in which to invest and diversify your portfolio. Detailed information regarding investment options and the Company Match will be provided at time of enrollment.

PAID VACATION

One of the ways Vertrue recognizes employee dedication and service is to provide paid vacation time to all regular full-time and part-time employees. Employees are eligible for ten days of vacation per calendar year; additional vacation time is given based on length of service.

PAID PERSONAL TIME

Full-time and part-time employees receive two paid personal days per calendar year. Personal time must be scheduled and approvedin advance.

PAID HOLIDAYS

Beginning on your employment date, employees receive 7 paid holidays and 3 floating holidays (of which 2 may be company designated) per year. Vertrue observes the following holidays: New Year’s Day Memorial Day Independence Day Labor Day Thanksgiving Day The Friday after Thanksgiving Christmas Day

TUITION ASSISTANCE

In support of employees’ continued education, Vertrue provides financial assistance for educational development through a tuition reimbursement program. All employees are eligible to participate. The maximum reimbursement per year is $3,750 for full-time and $1,500 for part-time employees.

DIRECT DEPOSIT

Vertrue employees may elect to have their bi-weekly paychecks directly deposited into a checking or savings account at the financial institution of their choice.

LONG-TERM CARE INSURANCE

This benefit is designed to help protect you and your family from the burden that is often associated with receiving Nursing Home or In-Home and Community Based Care. Vertrue offers full-time employees the opportunity to purchase Long-Term Care Insurance on yourself, your spouse, parents, parents-in-laws, grandparents, and/or grandparents-in-law.

PRODUCT MEMBERSHIPS

Employees receive Complimentary Memberships in the company's money-saving programs. With these memberships, employees have access to thousands of dollars in savings each year in products and services.
